Consent to the Processing of Personal Data - Wearables

1. I hereby grant the business entity INTELEK spol. s r.o., ID No.: 4944611, with its registered office at Olivova 2096/4, Nové Město, 110 00 Prague 1, registered in the Commercial Register maintained by the Regional Court in Brno, Section C, File 12338, newly renamed INTELEK servis s. r.o., ID No.: 4944611, with its registered office at Olivova 2096/4, Nové Město, 110 00 Prague 1, registered in the Commercial Register maintained by the Municipal Court in Prague, Section C, File No. 353200, and the successor company INTELEK.CZ s.r.o., ID No. 14089459, with its registered office at Olivova 2096/4, Nové Město, 110 00 Prague 1, registered in the Commercial Register maintained by the Municipal Court in Prague, Section C, File No. 360411 (hereinafter referred to as the “Controller”), consent, within the meaning of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016, on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (hereinafter referred to as the “General Data Protection Regulation”), in connection with the operation of the iGET GO and iGET GO Plus applications, to process the following personal data
:- email address,
- gender,
- date of birth,
- height and weight data,
- information related to your athletic performance and daily routine (such as number of steps, heart rate and blood pressure, sleep patterns and duration, calories burned, walking, cycling, swimming, etc.),
- information about a specific device (such as hardware model, operating system version, unique device identifiers, and mobile network information, including phone number).

2. This data will be processed for the following purposes
:- backing up to cloud storage,
- anonymized personal data for the purpose of evaluation and comparison with other app users; we may subsequently display this anonymous evaluation to you in the app,
- email address for the purpose of advertising and marketing campaigns, i.e., primarily sending commercial communications.

3. This data will be processed by the Controller for as long as you use the Controller’s services. For direct marketing purposes, the Controller will continue to process personal data for a period of 5 years, or until you revoke this consent. The Controller will process personal data both manually and automatically, either personally or through its employees. The following processors will also process personal data for
advertising and marketing purposes:- the operator of the cloud storage service Satomar, s.r.o., ID No.: 29201586, with its registered office at Šimáčkova 247/31, Líšeň, 628 00 Brno.

4. You hereby grant your explicit consent to the above-mentioned processing. Consent may be withdrawn at any time, for example by sending an email to remove@intelek.eu or a letter to the contact address of INTELEK.CZ, Ericha Roučky 1291/4, Černovice, 627 00 Brno.

5. Please note that under the General Data Protection Regulation, you have the right to
:- withdraw your consent at any time,
- request information from us regarding what personal data we process,
- to request an explanation from us regarding the processing of personal data,
- to request access to this data from us and have it updated or corrected,
- to request the deletion of this personal data,
- to contact us or the Office for Personal Data Protection in case of doubts regarding compliance with obligations related to the processing of personal data.

6. By granting this consent, I declare that I am aware that the performance of the contract is not contingent upon the granting of this consent and that I am granting this consent of my own free will.
